These questions come from family members, adult children, spouses, and caregivers who are navigating one of the most difficult seasons of life. We have tried to answer them the way we would if you were sitting across from us — honestly, warmly, and without medical jargon.
What is an end-of-life doula, and how is that different from a hospice nurse?
A hospice nurse is a licensed medical professional who manages pain, medications, and clinical care. An end-of-life doula is a non-medical companion who focuses on the emotional, spiritual, and practical needs that clinical schedules often cannot accommodate.
We sit with your loved one, hold space for difficult conversations, help with legacy projects, guide family members through what to expect, and simply make sure no one faces this journey alone. We work alongside — never instead of — your hospice or medical team.
Do I have to be on hospice to use your services?
No. Sacred Passage serves families at any stage of serious illness — whether your loved one has just received a serious diagnosis, is in the middle of treatment, is on hospice, or is in the final days of life. You do not need a hospice referral or a physician's order to contact us.
Many families reach out to us months before hospice is even discussed, simply because they want a steady, informed presence to help them navigate what lies ahead.
Is Sacred Passage a licensed medical or clinical service?
No. Sacred Passage Group LLC is a private-pay, non-medical companion service. We do not diagnose, prescribe medications, administer treatments, or replace any licensed clinical provider.
Our founder holds credentials as a Registered Nurse and a retired Air Force Technical Sergeant, which means our team brings clinical awareness and professionalism to every interaction — but our role is always that of a compassionate guide, not a clinician.
What does a typical visit look like?
Every visit is shaped by the needs of the person and family we are serving. A visit might include:
• Quiet sitting and presence at the bedside
• Guided conversation about life stories, memories, or wishes
• A legacy activity such as recording a voice message or writing a letter
• Coaching family members on what physical signs to watch for
• Coordinating questions to bring to the hospice or care team
• Prayer, meditation, music, or reading — based on the individual's wishes
Visits are typically 60–90 minutes for companionship packages, and up to 12 or 24 hours for vigil support.
What is vigil support, and when should I call you?
Vigil support means a doula stays present at the bedside as death approaches — providing quiet companionship for the person dying, and relief for family members who need to rest, eat, or step away without leaving their loved one alone.
You should call us when:
• Your loved one's hospice team has said that active dying may be days or hours away
• Your family is exhausted and cannot sustain continuous bedside presence
• Your loved one has expressed a wish not to die alone
• You want a calm, experienced presence to guide you through this sacred time
We offer a Thanatos Activation plan ($125) specifically for families who want to be on our priority call list before a crisis moment arrives.
We are not religious. Can you still help us?
Absolutely. Sacred Passage is spiritually sensitive but not religiously affiliated. We honor whatever belief system —or none at all — guides your family. Some families want prayer, scripture, and ritual. Others want quiet, music, nature, or simply a human presence. We follow your lead entirely.
We have served families of many faiths as well as secular and non-religious families, and we adapt completely to your values and preferences.
My loved one has dementia. Can you still work with them?
Yes. Many of the families we serve include individuals living with dementia or memory loss. Our approach is presence-based — meaning we do not require conversation or cognitive engagement to provide meaningful support.
We sit, we hold hands, we play familiar music, we read aloud, we create sensory comfort. For families, we also offer coaching on communication techniques that reduce distress for someone with dementia.
What if my loved one passes away during a vigil?
If your loved one passes during a vigil, our doula remains with you. We do not rush you. We help hold the space for as long as your family needs before contacting the hospice nurse or funeral home. We can help facilitate the notifications that need to happen, sit quietly with family members, or simply be a grounded presence in those first moments after a death. You will not be alone.
Can Sacred Passage help with bereavement after my loved one passes?
Yes. We offer Kardia™ Bereavement support ($250–$1,200) — a non-clinical grief support program for the early months following a loss. This is not psychotherapy; it is compassionate, structured companionship designed to help
you feel less alone as you process grief.
For clinical mental health support, we will always refer you to a licensed therapist or grief counselor.
What areas do you serve?
We primarily serve Greater Houston and Harris County, Texas for in-person visits and vigil support. Virtual support (video visits, telephone check-ins) is available to families throughout Texas and beyond.

How do I pay? Do you accept insurance or Medicare?
Sacred Passage is a private-pay service. We do not currently accept Medicare, Medicaid, or private insurance for our doula and companionship services. Some families are able to use:
• Health Savings Accounts (HSA) or Flexible Spending Accounts (FSA)
• Long-term care insurance (check your specific policy)
• Veterans' benefits in certain circumstances
• Sliding scale or payment arrangement discussions on a case-by-case basis

How quickly can a doula be available after I call?
For planned companion visits, we typically schedule within 2–5 business days. For vigil and urgent needs, availability depends on our current calendar — which is exactly why we offer the Thanatos Activation™ plan: a priority placement that puts your family at the front of our vigil list so we can respond much more quickly in a crisis.
I live out of state and my parent is in Houston. Can I still work with you?
Yes. We regularly work with long-distance family members. We can be your eyes and ears at the bedside, provide you with regular updates (with your loved one's consent), and help bridge the distance so you feel connected and informed even when you cannot be physically present.
